Monday - Friday, 8 am - 5 pm
22 - 23 patients per day
Chronic care management -- COPD, kidney disease, heart disease, diabetes
Wellness exams, physicals, and chronic pain management
Hospital privileges required
Adult population -- ages 18 and up
