Job description
Sumo Medical Staffing is seeking an experienced Nurse Practitioner for a PACE (Program of All-Inclusive Care for the Elderly) locum assignment in Grand Junction, Colorado. This role provides comprehensive care to elderly patients within a clinic-based and community-focused healthcare model.

Work Environment & Schedule Schedule: 4x10-hour shifts
No holiday coverage required
Primary Care Clinic setting
In-home visits as needed
Interdisciplinary PACE care team environment
Clinical Scope Primary care management
Preventive care
Urgent care services
Assessments across multiple care settings
Community referral coordination
Collaboration with PACE team members
Additional Requirements Active, unrestricted Colorado NP license
Board Certified
Minimum 2 years of experience
DEA
ACLS
CV in MM/YYYY format with all work and education gaps explained
