We are seeking a dedicated and compassionate Licensed Practical/Vocational Nurse (LPN/LVN) to join our team at Avir at John Jones. As an LPN/LVN, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our patients.
About the Role:
As an LPN/LVN, you will be responsible for providing direct patient care under the supervision of a licensed nurse. You will work closely with our multidisciplinary team to develop and implement individualized care plans, monitor patient progress, and maintain accurate and up-to-date medical records. Your exceptional communication skills will enable you to effectively educate patients and their families on treatment options, medication, and other essential health information.
Responsibilities:
Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
Develop and implement individualized care plans in collaboration with the healthcare team
Monitor patient progress, identifying and reporting any changes or concerns to the nursing team
Maintain accurate and up-to-date medical records, including patient intake, treatment, and discharge information
Communicate effectively with patients, families, and healthcare team members to ensure seamless patient care
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:
Current LPN/LVN license in the state of Texas
