Job Description Job Description The LPN at The Madison Health and Rehab plays a critical role in providing comprehensive nursing care to patients, primarily managing a caseload of up to 15 patients. This position operates within both nurse-only teams and multidisciplinary healthcare teams, ensuring high-quality care through close coordination and communication.
$25.50 per hour
Monday - Friday 7a -3p
Full time plus benefits
Responsibilities
Provide direct patient care including basic nursing services
Administer medications safely and accurately
Monitor and record vital signs regularly
Perform wound care following established protocols
Maintain detailed and accurate patient documentation
Educate patients and families on health management
Coordinate care with other healthcare professionals
Communicate effectively within nurse-only and multidisciplinary teams
Preferred Qualifications
Current LPN license
