Job description
Evergreen Woods, a premier Life Plan Community in Branford, CT, is hiring an LPN to provide compassionate nursing care for older adults in assisted living, memory support, and skilled nursing. The LPN will administer medications and treatments, monitor resident conditions, document care, and support physicians and RNs in implementing care plans. This role helps coordinate with families, reinforce safety and infection-control practices, and support wellness and lifestyle programs while modeling our resident-first culture of respect, teamwork, and attentive service.
