Job description
Humana is seeking a Patient Services Coordinator-LPN to schedule visits and coordinate care among field staff, patients, and physicians. The role includes maintaining logs and supporting daily office and staff operations to ensure continuity of care.
The ideal candidate is an LPN with at least 1 year of home health experience, strong communication, and the ability to manage multiple tasks efficiently. Valid Driver's license required; occasional on-site visits may be needed.
