Job description
CenterWell is seeking a Patient Services Coordinator-LPN to schedule visits and coordinate with field staff, patients, physicians, and care teams to ensure continuity of care. The role also supports day-to-day office and staff management.
Responsibilities include managing patient schedules, handling coverage duringabsences, submitting infection control forms, and assisting with weekly case conferences. A valid nursing license and home health experience are required.
