We are seeking a highly motivated LPN Shift Supervisor to join our team at Gurwin Jewish ~ Fay J. Lindner Residences, a luxurious and elegant Long Island assisted living community.
About the Role
$10,000 Sign On Bonus
Tuesday to Saturday - 3:00p.m. - 11:15p.m.
As an LPN Shift Supervisor at Gurwin Jewish ~ Fay J. Lindner Residences, you will be responsible for ensuring the highest level of care and service to our residents. Your duties will include:
Conducting daily rounds to ensure resident care is being provided in accordance with established protocols
Identifying and addressing resident needs, concerns, and complaints
Maintaining accurate records and reports of resident care and service
Participating in Quality Improvement initiatives to ensure continuous improvement of care and services
Collaborating with other departments to ensure seamless transitions and communication

Responsibilities
Supervise and coordinate the work of LPNs and HHAs to ensure efficient and effective delivery of care
Conduct daily rounds to ensure resident care is being provided in accordance with established protocols
Maintain accurate records and reports of resident care and service
Participate in Quality Improvement initiatives to ensure continuous improvement of care and services
Collaborate with other departments to ensure seamless transitions and communication
Requirements
Current LPN licensure in the State of New York
Minimum of two years of experience in long-term care or assisted living
Strong leadership and communication skills
Ability to work effectively in a fast-paced environment
Proficiency in electronic medical records (EMRs)
