Hartford Healthcare is seeking a compassionate LPN (Licensed Practical Nurse, Visiting Nurse) to deliver high-quality in-home care to patients across Connecticut. In this role, you will provide direct nursing care, administer medications, monitor patient conditions, and follow individualized care plans under RN and provider direction. You’ll educate patients and families, document accurately in the EMR, and collaborate with an interdisciplinary team.
