Job Description Job Description The LVN Home Health professional provides essential nursing care to patients in a home health setting, ensuring their comfort, safety, and well-being. This role involves administering medications, monitoring vital signs, managing wound care, educating patients and families, and maintaining accurate documentation while effectively communicating with the healthcare team.
Responsibilities
Provide direct patient care and support in the home
Manage and administer medications as prescribed
Perform wound dressing and monitor wound healing progress
Monitor and record vital signs regularly
Educate patients and families about care plans and health management
Document patient information and clinical data accurately
Communicate effectively with patients, families, and healthcare team members
Preferred Qualifications
At least 1 year experience in licensed vocational nursing
Skilled in wound care and medication administration
Proficient in monitoring vital signs and infection control practices
Familiarity with electronic health records (EHR)
Strong communication, empathy, and time management skills
