Home Health LVN provides skilled nursing care to patients in their homes under the direction of an RN and in accordance with the patient's established plan of care.
We are looking for an LVN who is professional, dependable, compassionate, and comfortable working independently in a home health environment.
Responsibilities
Provide skilled nursing care according to the patient's plan of care
Monitor and document patient condition, vital signs, and changes in health status
Administer medications and treatments as ordered and within LVN scope of practice
Perform wound care and other authorized nursing procedures as assigned
Assist with disease management and patient education
Maintain accurate and timely clinical documentation
Communicate changes in patient condition to the supervising RN and appropriate clinical team members
Follow physician orders and established care plans
Maintain infection-control and patient-safety standards
Protect patient privacy and confidentiality
Follow all P&A Recruiting and client policies and procedures
Maintain professional communication with patients, families, and healthcare team members
Requirements: Current and unrestricted Texas LVN license
Current CPR/BLS certification
Valid driver's license and reliable transportation
Ability to travel to assigned patient homes
Strong communication and documentation skills
Ability to work independently and follow an established plan of care
Home health experience preferred
Must successfully complete all required onboarding and background-screening requirements
