Job description
Privia Health is seeking a Registered Nurse Care Manager to join our Maryland market. The role partners with Primary Care providers to identify, screen, track, monitor, and coordinate care for patients with multiple chronic conditions, delivering Comprehensive Collaborative Care in a patient-centered model across in-person and virtual settings.
This position requires 25% travel, strong EHR skills, and a MD/DO license or License LPN, with experience in population health and care coordination;
