Job description
The Paramount at Somers Rehabilitation and Nursing Center is seeking a detail-oriented MDS Assessor to support accurate resident assessments and individualized care planning. You’ll work with residents and the interdisciplinary care team to gather, document, and communicate information about residents’ needs.
Responsibilities include conducting and documenting assessments on schedule, collecting information via record reviews and interviews, completing MDS and RAI documentation, sharing findings
