MDS Assessor
The Paramount at Somers Rehabilitation and Nursing Center
Salary: Up to $125,000 per year
The Paramount at Somers Rehabilitation and Nursing Center is seeking a detail-oriented MDS Assessor to support accurate resident assessments and individualized care planning. You’ll work with residents and the interdisciplinary care team to gather, document, and communicate information about residents’ needs.
Responsibilities
Conduct and document resident assessments in accordance with applicable requirements and timelines.
Gather accurate information through record review, resident and family interviews, and collaboration with care team members.
Complete assigned MDS and Resident Assessment Instrument (RAI) documentation.
Share assessment findings with the interdisciplinary team to support individualized care planning.
Monitor assessment schedules and follow up on missing or incomplete information.
Maintain accurate, timely records and protect confidential resident information.
Assist with assessment-related audits, surveys, and quality improvement activities.
Qualifications
Experience with MDS assessments and the RAI process in a skilled nursing or long-term care setting preferred.
Nursing or clinical experience preferred.
Familiarity with resident documentation, care planning, and long-term care requirements.
Strong organization, communication, and attention to detail.
Ability to meet deadlines and work collaboratively across departments.
Compensation
Salary of up to $125,000 per year , depending on qualifications and experience.
