University Park Rehabilitation and Healthcare Center in Fort Wayne, IN seeks an MDS Coordinator / Care Plan Nurse to ensure accurate and timely Medicare/Medicaid MDS and PPS documentation, integrating nursing, dietary, social recreation and rehabilitation services to optimize reimbursement.

You will assess new admissions, manage MDS schedules, enter data, coordinate interdisciplinary teams, and prepare care plans that reflect patient needs and regulatory requirements.
