Job description
University-Park-Rehabilitation-and-Healthcare-Center in Fort Wayne, IN is seeking an experienced MDS Coordinator / Care Plan Nurse to ensure timely completion of Medicare/Medicaid MDS documents and accurate PPS submissions.
This role integrates nursing, dietary, social recreation, restorative and physician services to optimize reimbursement and resident care plans.
Requires current RN license; LTC experience preferred; strong data entry and regulatory knowledge; on-site position.
