Job description
Heritage Health Care Center, part of HHP Lifeshare in Minster, OH, is seeking an MDS Coordinator, RN to lead our assessment and care-planning process for individuals with developmental disabilities. In this role, you will coordinate and complete MDS assessments, ensure regulatory compliance, and support accurate documentation for quality outcomes and reimbursement. You’ll collaborate closely with an interdisciplinary team to develop person-centered care plans, mentor staff on documentation standards, and champion our culture of respect, dignity, and independence for those we serve.
