Job description
Otterbein SeniorLife in Ohio seeks an experienced MDS Nurse Transition Leader to oversee MDS duties and care plan development across locations. You’ll coach staff, coordinate with coordinators, and lead Medicare/therapy meetings while upholding person-centered care.
The role requires at least 5 years of MDS experience in long-term care, a BSN, strong PDPM/casemix skills, and proficiency with EMR/PCC. Travel up to 100% may be necessary; Otterbein offers comprehensive benefits and growth
