Job description
South Mountain Rehab Center in Boonsboro, MD, is seeking a dedicated MDS RN Coordinator to manage Minimum Data Set assessments and develop comprehensive care plans. You will review medical records, collaborate with physicians and families, and ensure accurate documentation to support patient outcomes.
The role requires at least 1 year of MDS experience; PCC experience is a bonus. This is a full-time position with competitive benefits including PTO and health coverage.
