Medical Assistant – Care CoordinatorCreate the future of e-health together with us by becoming a Medical Assistant – Care Coordinator.At CompuGroup Medical we have the mission of building ground-breaking solutions for digital healthcare. Our vision is revolutionizing how healthcare professionals produce, access, and utilize information and thus enabling them to focus on the core value of their work: patient outcomes.We are seeking a motivated and patient-focused Care Coordinator to support patients enrolled in Medicare's Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.This is not a traditional back-office Medical Assistant role. Instead, you will serve as a key point of contact for high-risk patients with multiple chronic conditions — helping them stay engaged, compliant, and on track with their care plans through structured outreach and monitoring. This on-site role offers a stable Monday through Friday schedule with no evening or weekend commitments.Your Contribution:Make 60+ outbound calls per day to eligible patients using our dialer systemAccurately document all call outcomes utilizing multiple client EHR and other systems.Conduct monthly check-in calls for RPM and CCM patientsReview and monitor synced vital signs from RPM devicesExplain RPM program benefits in clear, patient-friendly languageSchedule and order RPM devices, educate patients on proper usage, and confirm patient contact details.Conduct structured health assessments and follow personalized care plans tailored to each patientProvide education on nutrition, exercise, medication adherence, and lifestyle choicesObtain and document verbal enrollment consent per program requirementsTrack and document patient progress accurately and consistentlyYour Qualification:Active National Certified Medical Assistant Required (To expedite the hiring process, please upload NCMA/NRMA credentials)Minimum 2 years of healthcare-related experienceMinimum 1 year in related Chronic Care Management servicesStrong communication and interpersonal skillsComfortable with high-volume outbound callingStrong documentation and EHR proficiencyWillingness to work on-site during regular business hours, Monday-FridayWhat you can expect from us:Salary: $20/hrMonday–Friday schedule: Enjoy a consistent Monday–Friday schedule with no night or weekend shifts.
