Medical AssistantHourly Pay Range: $19.89 - $28.84 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.Position Highlights:Location: Warrenville, ILFull Time: 40 hours/weekHours: Monday-Friday, 8:30a-5:00pWhat you will need:The Transitions of Care Navigation Program follows high-risk patients discharged from Inpatient, Observation, or Emergency Department. Patients are contacted telephonically up to thirty (30) days post-discharge.
