Job description
WelbeHealth is seeking a Medical Director I to lead comprehensive in-person assessments and coordinate with the IDT to create individualized care plans for participants. This role oversees care in nursing homes and hospitals, including admissions and discharges, and supports Nurse Practitioners within a value-based model.
Responsibilities include providing continuous medical coverage, engaging with community physicians, and participating in 24-hour care coordination.
