This is an exceptional opportunity for a mission-driven physician to combine clinical excellence, strategic leadership, quality improvement, and interdisciplinary collaboration to help frail older adults remain safely living in their homes and communities.

As a PACE organization, we provide comprehensive, coordinated healthcare and supportive services to medically complex older adults who qualify for nursing home level care but wish to remain living in their communities.

Our interdisciplinary model empowers physicians to practice medicine the way it was intended: with time to collaborate, focus on outcomes, and build meaningful relationships with participants, families, and caregivers.
