Medical Management Specialist I | 3+ Years of Experience | Onsite/Field-Based | Chicago Area, IL | Contract
Walker Healthforce is seeking a Medical Management Specialist I with 3+ years of experience to provide medical management, health education, and care coordination support for members participating in government healthcare programs. This is a Contract opportunity.
Start Date: ASAP
Location: Onsite/Field-Based - Chicago Area, IL
Hours/Schedule: Monday-Friday, 40 hours
Worker Type: W2
Core Requirements:
Current, valid, unrestricted RN, LPC, or LMSW license, as applicable
RN candidates require 3 years of direct clinical care experience; LPC/LMSW candidates must have the clinical practice required for applicable licensure
3 years of wellness or managed care experience presenting clinical issues with members and physicians
Behavioral health and physical health experience
Medical data analysis experience
Strong verbal and written communication skills
Proficiency with Word, Excel, PowerPoint, databases, and web-based applications
Current driver's license, transportation, applicable insurance, and ability to travel within an assigned territory
We Consider It A Bonus If You Also Have: 3 years of clinical experience
Patient education or Condition Management experience
Transition of Care experience
Experience managing complex or catastrophic cases
Bilingual English/Spanish skills
Certification in Case Management, Training, Project Management, or a nationally recognized healthcare certification
Job Responsibilities: Conduct medical management and health education programs
Support members with behavioral and physical healthcare needs
Gather and analyze medical data for regulatory reporting
Communicate medical needs with members and physicians
Coordinate with internal teams, external vendors, and community resources
Represent the organization when working directly with members
Travel throughout an assigned Chicago-area territory
