Job description
Oak Street Health is seeking a Medical Social Worker to support psychosocial needs of patients within our centers. You will conduct assessments, advocacy, and care coordination to high-risk patients and their caregivers to improve health outcomes.
Responsibilities include comprehensive assessments, care planning with the inter-disciplinary team, navigation of insurance benefits, and education on support options. Fluency in Spanish, Polish, or Russian is preferred; US work eligibility required.
