Job description
CMH Home Health Care, part of LHC Group, a nationwide post-acute care partner, seeks a Medical Social Worker to provide medical social services under physician direction and with the Interdisciplinary Group to aid understanding of social and emotional factors affecting health and coping strategies.
You will help develop a Plan of Care, educate the care team on community resources, and, if needed, coordinate bereavement services with clear service delineation.
