Job description
CMH Home Health Care, part of LHC Group and UnitedHealth Group, is seeking a Medical Social Worker to provide medical social services under physician direction and an Interdisciplinary Group to help patients and families understand social and emotional factors affecting health status.
The role includes developing a Plan of Care with the care team, instructing staff on community resources, and serving as Bereavement Coordinator to address bereavement needs within the patient/family context.
