Job description
CMH Home Health Care, part of LHC Group and the Optum family, seeks a Medical Social Worker to provide social work services under physician direction to address significant social and emotional factors impacting patient health. You will join an interdisciplinary team and help develop a comprehensive plan of care.
You will coordinate community resources and, when needed, serve as Bereavement Coordinator, ensuring appropriate bereavement services are provided to patients and families.
