Job description
Kaiser Permanente is seeking a Medical Social Worker II to support patients and families in coping with social, emotional, and practical aspects of illness. You will assess, plan, and coordinate care in hospital and clinic settings, ensuring resources and referrals meet complex needs.
In this role you collaborate with a multidisciplinary team, deliver counseling, develop treatment plans, and navigate community and Kaiser resources to optimize patient outcomes and continuity of care.
