Job description
Health Advocates Network, Inc. is seeking a Social Worker to support patients across partner hospitals and clinics. In this role, you will complete psychosocial assessments, develop care plans, and coordinate services to support safe discharges and continuity of care. You’ll provide counseling, crisis intervention, and resource navigation for patients and families, collaborating with nurses, physicians, and case managers.
