Job description
Integrated Care Team PositionThis position serves as an integrated part of the Patient - Aligned Care Team (PACT) within Community Care and works collaboratively with other members of the team to provide a robust interdisciplinary approach to care. Interfaces and collaborations also occur with larger health care system providers including VA and non-VA specialists, hospitals, diagnostic and treatment facilities, and community-based programs.
