Job description

Shift: 8:00 AM - 5:00 PM
Experience:
- 12 months of experience with 3 months within the last 12 months.
- 2 years of Medical Surgical Unit experience.
- 3 months of RPMS (Resource and Patient Management System) experience is preferred.
Requirements:
- Candidates must have a Arizona license or compact license (required for submission).
- This role will require floating to multiple units
- Local & travel allowed.
- Flu vaccination required after submission. Religious, medical, and personal declinations accepted.
- Anyone currently working at this facility cannot be submitted.

Additional Details:
Required Skills/Experience: Two (2) years or more of discharge planning experience required.
Special Requests: Flu declinations accepted on condition traveler wears a mask at all times. Housing is limited.
Unit Details: Performs case management and coordinates discharge planning for age-appropriate inpatients with chronic diseases and special health care needs across the Multi-Service, Intensive Care, OB, and Adolescent Care units. Maintains a tracking system for patients referred for admission to other healthcare facilities, performs chart reviews for requested services, and coordinates return to the facility or discharge home.
Shift & Scheduling: Monday through Friday. 15-minute call response time for all units with a call requirement.
14 days maximum time off
1 Charge/Lead/Supervisor reference required from within the last 12 months
