Job description

Community Care Givers in Grand Rapids, MI seeks an experienced Registered Nurse (RN) to deliver Medicare-certified home health services and complete SOC admissions and OASIS assessments across the Grand Rapids area.

Responsibilities include SOC/ROC visits, comprehensive nursing assessments, and developing Plans of Care, with emphasis on accurate documentation and patient education. Strong Medicare SOC experience preferred.
