Job description
Oak Street Health is seeking a Nurse Practitioner/Advanced Practice Provider to deliver high-quality primary care to adults on Medicare in underserved communities in Charleston. In a patient-centered, team-based setting, you will lead comprehensive assessments, manage chronic conditions, provide preventive care, and coordinate with physicians, nurses, and social workers to support care plans.
You will educate patients and families, and use data and EMR tools to improve outcomes.
