Job description
Trilogy Health Services is seeking a Memory Care Leader (RN) to oversee the Legacy Lane memory care neighborhood, ensuring dementia care practices are implemented and resident wellbeing is prioritized.
You will collaborate with the Executive Director, educate campus staff and families, develop individualized Service Plans, and maintain a dementia-safe environment while guiding staff through the care continuum.
