Job description
Duties
Perform difficult and complex assessments; conduct interviews; evaluate and analyze health care plan and resources; and plan, coordinate, and monitor the delivery of services.
Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient.
Provide individual and group support, psycho education, and skills training on topics such as stress management, coping mechanisms, and relapse prevention.
Develop personalized safety plans with patients, integrating these into the medical treatment plan.
Develop, implement, communicate, and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
