Job description
TriStar Health seeks a Mental Health Intake Nurse to assess behavioral health patients at point of entry. Responsibilities include conducting psychosocial and risk assessments, gathering medical and psychiatric histories, triaging acuity, and coordinating safe admissions, transfers, or referrals. The nurse collaborates with psychiatrists, ED and inpatient teams, and social workers to develop initial care plans, ensure regulatory and documentation accuracy, educate patients and families, and support a safe, therapeutic environment aligned with evidence-based and trauma-informed practices.
