nAbout the Role:
nWe're seeking a Mental Health Practitioner to support our growing organization. A Mental Health Practitioner is responsible for providing treatment programs and support to patients in the comfort of their own home. The right candidate will be capable of working in a fast-paced environment, using their knowledge of psychiatry to assess patients and create individualized treatment programs and support plans. A successful Mental Health Practitioner will be detail oriented and able to work independently, using organizational skills to manage their schedule and prioritize tasks on a daily basis.
nResponsibilities:
nConduct comprehensive assessments to develop personalized treatment plans.
nProvide individual and group therapy sessions using evidence-based practices.
nMonitor client progress and adjust treatment plans as necessary.
nCollaborate with a multidisciplinary team to ensure holistic care.
nMaintain accurate and timely documentation of client interactions.
nEducate clients and families on mental health issues and coping strategies.
nParticipate in regular team meetings and professional development opportunities.
nAdvocate for clients' needs and connect them with community resources.
nRequirements:
nMaster's degree in Social Work, Counseling, Psychology, or related field.
nCurrent state licensure as a Mental Health Practitioner (LPC, LCSW, etc.).
n2+ years of experience in a clinical setting preferred.
nStrong understanding of mental health disorders and treatment modalities.
nExcellent communication and interpersonal skills.
nAbility to work collaboratively in a team-oriented environment.
nCompassionate demeanor and a commitment to client-centered care.
nProficient in electronic health record (EHR) systems.
