Job TitleAllegheny Health NetworkJob DescriptionGeneral Overview:Maintains safety and structure by observing and recording patient information pertinent to the treatment plan and reports information to appropriate staff. Works under nursing supervision.Essential Responsibilities:Collaborates with the multi-disciplinary team to provide for an on-going assessment of changes in patient status and gathers pertinent patient care data through rounding and patient interaction using all available resources in the inpatient or outpatient setting. (60%)Organizes patient care tasks according to priority and importance of tasks to be completed. (20%)Obtains vital signs and height and weight measurements. Provides crisis intervention on the unit to maintain safety. Responds to facility Code events. (10%)Reports any changes in physical, mental and emotional conditions to nursing staff and suggests modifications to the master treatment plan.
