Job description

Join a team that shares your calling

At MultiCare, you're more than just a job title - you're part of a team built on trust that cares for each other, our patients and our communities. Belonging here means living our mission and values every day. If your purpose and passions align with ours, you'll find a place to grow, do meaningful work and build a career you love in a community that feels like home.

**This is not a traditional outpatient therapy role. The position is based in a residential behavioral health setting and combines therapeutic services with hands-on case management, care coordination, crisis intervention, and support for clients as they build the skills needed for long-term recovery and community integration.**

FTE: 32 hrs/wk, Shift: Day 9:00am-5:30pm, Schedule: Wednesday - Saturday

The Intensive Community Support and Recovery Program (ICSRP) is dedicated to helping adults with chronic and persistent mental illnesses, including schizophrenia, schizoaffective disorder, bipolar disorder, and co-occurring mental health and substance use disorders, live meaningful and stable lives within their communities.

Our multidisciplinary team provides intensive case management, individual therapy, medication support, crisis intervention, and care coordination through a recovery-oriented and trauma-informed approach. We partner with clients to build on their strengths, develop practical life skills, access essential resources, and overcome barriers to recovery.

Working at ICSRP means making a tangible difference every day. Our services help individuals maintain stable housing, improve their health and independence, and reduce the need for psychiatric hospitalizations, incarceration, and long-term institutionalization, including admissions to Western State Hospital. We are committed to meeting clients where they are, fostering hope, and supporting each person's journey toward recovery and community integration.

Key Responsibilities: Administer medications safely and accurately in accordance with physician orders, agency policies, and applicable regulations.

Complete timely, accurate, and thorough clinical documentation, including progress notes, assessments, treatment plans, and treatment updates.

Conduct clinical assessments and provide therapeutic interventions based on clients' individual needs and treatment goals.

Utilize de-escalation techniques and crisis intervention strategies to maintain a safe, supportive, and therapeutic environment.

Facilitate weekly therapeutic groups using evidence-based, recovery-oriented practices.

Provide ongoing case management services, including care coordination, treatment planning, referrals, resource linkage, and client advocacy for an assigned caseload.

Assist clients with activities of daily living (ADLs) while promoting independence, dignity, and recovery.

Monitor clients' mental and physical health status, report significant changes, and collaborate with the interdisciplinary treatment team to ensure appropriate care.

Develop and maintain therapeutic relationships with clients through a trauma-informed, person-centered approach.

Collaborate with medical providers, therapists, case managers, and community partners to ensure comprehensive and coordinated client care.
