Job description
SCAN Health Plan seeks a Nurse Practitioner or Physician Assistant to deliver primary care to members in Portland-area residences and senior living communities. The role includes exams, vaccines, chronic disease management, and care coordination to prevent hospitalizations.
You will travel up to 75% in the assigned area, working in a remote/field-based model. A strong geriatrics background and EMR documentation proficiency are essential.
