Hartford HealthCare’s Neighborhood Health program is seeking per diem Advanced Practitioners to join our primary care team focused on improving health equity in underserved communities. This role offers mobile and community-based services to deliver high-quality preventative and urgent care across Connecticut.
Open to Family Nurse Practitioners and Physician Assistants, the position requires travel to home base hubs in Hartford and New Britain, then to clinic sites and Street Medicine, with 100%
