Job description
Wells Health seeks a Primary Care NP to provide comprehensive medical management for residents in Assisted Living Programs across multiple sites in the Valley Stream area. The role emphasizes treating in place to reduce ER transfers and enhance geriatric quality of life.
You will perform exams, manage chronic diseases, coordinate care with staff and families, and travel daily between Utica cluster sites, ensuring NYS DOH compliance with ALP regulations.
