Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations, traveling up to 100% across a defined coverage area. Each 45‑minute visit includes a medication and medical history review, physical evaluation, and optional diagnostic tests.
You will work with an intuitive iPad workflow and may travel locally or across licensed states; a board‑certified NP/PA with active licenses is required.
