Job description
GBMC HealthCare's Gilchrist Mobile Hospice program seeks a Nurse who orchestrates admissions from hospital referrals, grows admissions, and assesses patients for hospice care. You will build external partnerships and guide discharge planning with hospital teams, delivering education and case management.
A Maryland RN with a Bachelor of Science in Nursing is required. You will collaborate across admissions, IPU, and hospice physicians, maintaining clinical standards and using EMR and MS Office
