Job description
Liberty Cares provides home-based health services and is seeking a Nurse Practitioner (CSNP) to conduct comprehensive health risk assessments and management for members in the Liberty Cares program. The role involves working with physicians, care coordinators, families, and other staff to deliver individualized care in home settings.
