Job description
CenterLight Health System seeks a Nurse Practitioner Supervisor for the Mobile Program to oversee clinical coordination and oversee care for participants across sites in the PACE network.
The NPS will liaise with community physicians, nursing homes, and plan staff to implement person-centered plans and manage urgent care needs while ensuring documentation and care transitions meet high standards.
