Job description
Practice beyond the walls ofa traditional clinic as a full-time Mobile Nurse Practitioner serving patientsthroughout San Gabriel Valley and East Los Angeles, California. In this role,you will bring high-quality, relationship-based care directly to patientsthrough home visits, community-based care, and virtual appointments whileworking within a patient-centered, value-based care organization focused onimproving outcomes and supporting long-term health.
Our care model is focused onquality outcomes, preventive care, proactive interventions, and coordinatedpatient support rather than high visit volume. You will enjoy a high level ofclinical autonomy while working alongside an interdisciplinary team dedicatedto helping patients achieve better health outcomes.
Position Highlights
Full-Time Schedule:Monday–Friday | 8:00 AM–5:00 PM
Competitive Base Salary: $130,159-$195,238
$10,000 Sign-On Bonus
Mobile Practice Setting:Home visits, community-based care, and virtual appointments
Mileage Reimbursement
Quarterly Incentive Plan:Opportunity to earn up to 10% of quarterly earnings

Qualifications
Active and unrestrictedCalifornia Nurse Practitioner license
Active DEA license
Current BLS certification
Strong clinical assessmentand diagnostic skills
Ability to practiceindependently while collaborating effectively with an interdisciplinary careteam
Commitment topatient-centered, preventive, and value-based care
Excellent communication andrelationship-building skills
Comfortable providing carein home, community, and virtual settings
