Job description
Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments. Assessments last up to an hour and cover medical history, medication adherence, and social factors to identify care gaps.
You will administer tests as needed and coordinate with PCPs, case managers and other team members to improve overall health outcomes. Travel to the Cisco, TX area is required, with flexible scheduling options.
