Avail Health is seeking a mobile Nurse Practitioner to perform in-person Chronic Disease Assessments and Health Risk Assessments for Medicare Advantage members in long-term care facilities across Southern West Virginia. You will document conditions, develop care plans, and collaborate with an interdisciplinary team.
